Q: What can I expect when I start using Wellpartner pharmacy for my Specialty prescriptions?

A: Wellpartner is a fully engaged member of each patient’s “care team.” We work closely with you to understand your specific condition, concerns and needs.

When working with an individual for the first time, Wellpartner performs a thorough, personalized initial assessment to establish a true care-based relationship. In every case, this assessment is done by a Clinical Pharmacist. Through this process we create a profile that includes:

Overall patient condition and risk assessment Complete list of all medications currently prescribed Comorbidities (if any) Any concerns or specific instructions regarding our service or shipments

Q: How will I know my prescriptions have been sent to and received by Wellpartner?

A: Someone from Wellpartner will contact you for your insurance information upon receipt of your prescriptions from your prescriber or clinic. They will verify your shipping address, insurance information, and determine how soon you need your medications.

Q: When will I receive my first order?

A: Wellpartner will begin processing your prescriptions as soon as they are received from your healthcare provider. However, they will not be able to ship orders until they confirm with you your shipping address and obtain payment information.

Q: Will Wellpartner contact me at other times?

A: Yes, a clinical pharmacist may contact you with a few questions and to review your medications. This is in addition to the initial assessment and profile we create before your first prescription is filled.

Also, a Wellpartner clinical pharmacist is available by phone 24/7 to answer any questions you may have.

Q: Will I receive 30 days or 90 days of medication?

A: Your prescriptions will be filled as written by your healthcare provider. In many cases, patients and their prescribers agree that 90 days is preferable.

Q: How are my orders shipped?

A: Your order will ship either through the US Postal Service or via FedEx.

Q: Will my order ship overnight?

A: Not automatically. Your order will only ship overnight if there is an immediate need for a medication, or it is a refrigerated product.

Q: What does the packaging look like?

A: The packaging is either a small white box, or a grey envelope. The return address simply states “Wellpartner”. There is no indication that your package has come from a pharmacy, or any indication that it contains medication.

Q: Will my orders require a signature?

A: Not always. However, some orders that are very expensive may require a signature.

Q: How do I order a Specialty prescription or refill?

A: There are four ways to refill a prescription with Wellpartner:

Online — Log in to www.wellpartnerpharmacy.com, go to My Home and select the Medicine Cabinet tab. Click the refill box next to the medication you want filled. Then click Add to Cart. Follow the instructions to complete the transaction so your order can be processed.

Phone — Call us or have your health care professional call: 1-800-473-3516

Fax — Have your health care provider fax your prescription to: 1–877-597-3070

Mail — You can send a refill order to:

Wellpartner, Inc.
PO Box 5909
Portland, OR 97228–5909

The quickest way to refill a Specialty prescription is to have your doctor call or fax your prescription to Wellpartner.

Be sure to include the following information:

Medication name
Patient’s name, address, and telephone number
Allergies (if any)
Prescription health plan information (name, member number, group number)
Method of payment (credit card name, number, expiration date)

Q: Do I have to have an email address to place an order via the web site?

A: Yes, if you are ordering from www.wellpartnerpharmacy.com, an email address is required. If you don’t have an email address, you can order by tele-
phone: 1-800-473-3516.

Q: What are your shipping charges?

A: There is no additional charge for standard shipping of your order. In some cases, if you wish to expedite your order and choose second day or next day shipping an additional charge may be added. Also, Second day or next day shipping applies only to the shipping time and does not include processing time prior to shipment.

Q: Can Wellpartner send prescriptions to an international address?

A: No. Wellpartner does not accept international orders, or ship to international addresses.

Q: How do I check the status of my order?

A: You can check your order status online at any time. Just log in, go to My Home and select the Order Status tab. Once there is a final price for a prescrip-
tion, that price will be displayed in the order status.

Q: How do I update my account information?

A: You can update your account information at any time. Go to wellpartnerpharmcy.com and log into your account. From the My Home page, select Edit

Your Profile in the gray box located under today’s date. Your profile contains account information, plan information, health information and payment infor-
mation.

Q: How do I access my prescription history?

A: Your prescription history is maintained in your secure, online Medicine Cabinet. The Medicine Cabinet, located on the My Home page, is a record of all prescription and non-prescription products you’ve ordered from Wellpartner, including those items you have ordered by telephone, fax, mail or the Inter-
net. It provides an easy way to keep track of your prescriptions and place refill orders. In addition, you can re-order non-prescription products using the information in your Medicine Cabinet.

Q: What are the automated calls I receive?

A: The automated calls are to let you better track the path of a prescription that is processing. You may receive these when a new Rx is received, if a prior authorization is needed, or if there is a problem with a form of payment. All of these are a way to communicate important information to you.

Q: What if my insurance plan does not cover a particular medication?

A: Wellpartner will act as a single point of contact for your pharmacy needs. If we cannot fill one of your prescriptions for any reason we will work with you and the appropriate parties – your prescriber and insurance plan – to make sure it goes to a pharmacy that can. We will work with you and assist with prior authorizations, patient assistance and discount programs, etc.

Q: What if I have a balance due on my account? Will this hold up an order?

A: No orders are shipped without a form of payment.
